Introducción

Probablemente en el presente milenio el estudio del genoma humano permitirá hallar defectos genéticos relacionados con las enfermedades del disco intervertebral y de la columna espinal y posiblemente actuaciones genéticas, transplantes de células embrionarias de la notocorda o técnicas de cirugía mínimamente invasivas guiadas por neuronavegadores a tiempo real sumados a nuevas tecnologías en biomateriales o efectivos factores de crecimiento óseo permitirán prevenir o restablecer las enfermedades del raquis.

Sin embargo en el momento actual las técnicas quirúrgicas de liberación neural y de fusión espinal tienen todavía plena vigencia.

En los últimos 50 años se han descrito técnicas de fusión espinal lumbar: F. P., P.L.I.F., A.L.I.F., T.L.I.F. Para su ejecución es necesario resecar elementos vertebrales posteriores y manipular el contenido del canal vertebral.

Material y método

Desde octubre de 1999, treinta pacientes han sido intervenidos en nuestro hospital con una fijación 360° mediante una E.P.L.I.F. Técnica desarrollada en nuestro servicio.

La E.P.L.I.F., permite una fusión intersomática lumbar sin necesidad de entrar en el canal vertebral precisando únicamente resecar una pequeña porción de la parte externa de la faceta inferior del segmento a fusionar.

Discusión

En esa comunicación presentamos los pasos técnicos necesarios para la ejecución de una E.P.L.I.F. Presentamos los resultados radiológicos y clínicos obtenidos con esta nueva técnica en nuestros pacientes.
