Objectives: The aging population has led to an increase in neurosurgical pathologies and spinal surgery in particular, in patients above 70 years age are also highly complex multisystem illnesses and diseases. The development of surgical techniques, experience and training of surgeons in spinal surgery and the use of technology as intraoperative CT neuronavegadores and recording potential, minimize the risk of complications inherent to the surgical procedure, reducing the time of surgery and hospital stay.

Material and methods: Retrospective where there have been 870 lumbar spine surgery in patients over 70 years in a period of four years (January 2011 to December 2014) by the same surgical team. The statistical parameters in age range, weight, height, surgical pathology and psychobiological habits were the same. The study population included 61% women and 39% men; The age range was 71 to 89 years. 58% of patients diagnosed with degenerative lumbar canal stenosis, lumbar kyphoscoliosis 34% and 8% lumbar spondylolisthesis. It was used for the implementation of intraoperative spinal stabilization system equipment O-arm, neuro and registration of potential.

Results: They have implemented a total of 5,220 pedicle screws with an average of 6 screws per patient in a universe of 870 patients; average age was 78 years. The mean duration of surgery was 145 minutes and the hospital stay was 5.3 days.

Conclusions: It is imperative to achieve excellent results proper diagnosis, purify and individualize treatment for each patient, the expertise and experience of the surgical team and the appropriate use of imaging technology and art record; this increases accuracy and enables the patient to have a less prolonged surgery and shorter hospital stay. Regarding the degree of satisfaction with the procedure was 95.3%, between fully and very satisfied.
