Hemorragia en un quiste simple de la glándula pineal. Presentación de un caso

Resumen

Se presenta el caso de una mujer de 50 años con una hemorragia en el seno de un quiste simple de la glándula pineal, que debuta con intensa cefalea, vómitos y signos radiológicos de hidrocefalia aguda, siendo tratada con una derivación ventrículo-peritoneal de urgencia y posteriormente con la resección total, mediante craniectomía suboccipital, vía supracerebelosa infratentorial. La localización de la lesión se estableció con TC y RM. El diagnóstico anatomopatológico resultó ser el de un quiste simple glial de la epífisis con hemorragia en su interior.

Se realiza una amplia revisión de la bibliografía y se estudia este tipo de lesión, que, por su comportamiento clínico y por su aspecto radiológico, produce controversias en el diagnóstico y en la elección del tratamiento más oportuno.

Se comenta la técnica quirúrgica empleada y su resultado y, finalmente, se analiza su naturaleza y su origen.

Summary

This is the case of a 50 year-old woman with an haematoma of the pineal gland producing severe headache, vomiting and radiological signs of acute hydrocephalus. The patient was treated with a ventriculo-peritoneal shunt on an emergency basis and later on she underwent operation by a supracerebellar infratentorial approach. The localization of the lesion was established with CT and MRI. The anatomopathological diagnosis was of an hemorrhagic glial cyst of the epiphysis.

A revision of the literature is performed. Because of its chemical behaviour and radiological appearance both the diagnostic work-up and the best method of treatment of this type of lesion are controversial.

The surgical technique used and the result are commented.
