3. Encefalitis granulomatosa por acantamoeba

Presentamos el caso de un paciente de 33 años con Síndrome de Inmunodeficiencia adquirido que debuta con un cuadro subagudo de cefalea, confusión y hemianopsia homónima izquierda. En los estudios radiológicos se apreció proceso expansivo occipital derecho que no respondía favorablemente al tratamiento antitoxo-plasma.

Tras realizar una biopsia cerebral detectaron en los estudios citológicos la presencia de amebas en el tejido cerebral. Se decidió la extirpación completa de la lesión en un segundo tiempo quirúrgico. El resultado anatomopatológico fue concluyente con granuloma por Acantamoeba. Tras iniciar tratamiento con Fluconazol y Sulfadiacina la evolución posterior fue favorable. Los cultivos de ameba fueron remitidos a la sección de enfermedades parasitarias del CDC en Atlanta confirmándose el crecimiento por Acantamoeba grupo II. El paciente a los dos años de la cirugía, realiza una vida normal totalmente independiente pero presenta una reducción del campo visual izquierdo.

Conclusiones

Este es el primer caso de un paciente con SIDA y encefalitis granulomatosa por ameba que sobrevive tras responder favorablemente al tratamiento.
