Resumen

Tras casi un siglo de utilización de diferentes pautas para el manejo de la parálisis facial postraumática, no se ha llegado a un acuerdo sobre las indicaciones quirúrgicas. El objetivo principal de este estudio fue el de encontrar alguna relación entre los detalles de la fractura observados con la TC de alta resolución y la aparición y evolución de la parálisis, así como conocer los resultados de la aplicación del protocolo utilizado en nuestro centro.

Realizamos un estudio prospectivo sobre 216 pacientes con trauma cerebral y fractura de base tratados durante 3 años consecutivos. Encontramos 111 fracturas de peñasco y las clasificamos, de acuerdo con la TC de alta resolucion, en 2 tipos de fracturas longitudinales, 3 tipos de transversas y un tipo de oblícuas O mixtas. La incidencia de parálisis facial fue del 20,7%. Los resultados fueron buenos (Grados I a III de House) en 91,3% de los casos, con sólo 2 indicaciones quirúrgicas. Uno de los tipos de fractura longitudinal mostró una relación estadisticamente significativa con la aparición de parálisis. De los parámetros estudiados, sólo el momento de inicio fue un factor pronóstico importante.

Discutimos nuestros resultados sobre la base de una revisión de la literatura y proponemos nuestras conclusiones, enfatizando la utilidad del manejo conservador de esta patologia.

After almost a century of being well known, there is still no consensus concerning to the management and surgical indications in patients with post-traumatic facial paralysis. The main objective of this study was to find a possible relationship between the details of the skull fracture provided by high-resolution CT scan and the onset and evolution of facial paralysis. A second aim was to validate the management protocol used in our department.

We performed a prospective study on 216 patients diagnosed during three consecutive years. We found 111 petrous bone fractures and classified them according to the high-resolution CT into two types of longitudinal, three types of transverse and one type of oblique or mixed fracture. We observed good results (House Grades I to Ill) in 91,3% of the patients, with only two surgical indications. One of the longitudinal types of fracture showed an statistically significant relationship with the onset of paralysis. Among all the parameters considered, only the time of onset revealed as an important prognosis factor.

We discuss our results and by reviewing the literature draw our conclusions. We emphasize the utility of conservative treatment in the management of this pathology.
