Resumen

La columna vertebral al ser una estructura anatómicamente estable y con numerosos puntos referenciables perfectamente distinguibles tanto en imágenes de scan-ner como en el campo quirúrgico, ha hecho que se la considere ideal para la cirugía guiada.

La aplicación de esta técnica a la cirugía convencional de fijación transpedicular ha marcado nuestro objetivo.

Durante el año 1999 se intervinieron 39 pacientes, 13 varones y 26 hembras, con una edad media de 47.18±14.12 años. Por patologías: en 9 pacientes predominó una discopatía degenerativa; la discopatía degenerativa se asoció con una listesis degenerativa en 7 casos y con una estenosis de canal en 8. En otros 8 pacientes predo minó de forma aislada una listesis degenerativa, mien tras que en otros 7 lo hizo una estenosis de canal. Todos los pacientes se intervinieron de forma protocolizada, utilizando como método de cirugía guiada por la imagen el sistema desarrollado por BrainLAB.

En tres pacientes no pudo realizarse el protocolo preestablecido. Aunque este pudo llevarse a cabo en 36 pacientes, sólo se obtuvo una navegación adecuada en 22 casos (61%). De los 22 casos intervenidos mediante cirugía guiada con un total de 116 tornillos, 108 (93%) se consideraron estrictamente intrapediculares. En los 14 pacientes en los que no se consiguió practicar una cirugía guiada, de los 76 tornillos implantados única mente en 65 (86%) se obtuvo una ubicación estricta mente intrapedicular.

Consideramos que la neuronavegación raquídea ofrece una exactitud y Habilidad que reduce el margen de error en la ubicación transpedicular de los sistemas de fijación vertebral.

Summary

The spinal column has been considered ideal for guided surgery due to its stable anatomical structure and notable reference points which are perfectly distinguishable both in computer tomography (C.T.) images and in the surgical field.

Our main objective was to apply this technique to the conventional transpedicular fusion surgery.

During the last year (1999), 13 males and 26 females, with an average of 47.1, + 14.1 years were operated because of degenerative discophathy (9 cases) degenerative discophathy associated to listhesis (7 cases) and spinal canal stenosis (8 cases). AU of them were operated according to a preestablished protocol using the Brain Lab Image Guided System.

The preestablished protocol could not be applied in 3 patients.

Of the 36 patitents, only in 22 cases (61%), a pro-perly navigation was obtained. In these cases 116 screws were used: 108 (93%) strictly intrapediculars.

In the other 14 patients, without Guided Surgery, it was used 76 screws: 65 (86%) were strictly intrapediculars.

In our opinion Guided Spine Surgery, offers an accuracy and reability to reduce the margin of error in the Transpedicular location of the Spine Fusion Systems.
