Resumen

A partir del desarrollo técnico de los sistemas valvulares en la década de los 50, comienza a tratarse la hidrocefalia mediante el uso de derivaciones ventrículoatriales (VA) y posteriormente ventrículo-peritoneales (VP) siendo estas últimas las más frecuentemente utilizadas. Si bien se trata de formas de tratamiento eficaces y bien toleradas no están exentas de complicaciones.

Se presenta un caso de complicación de válvula VP por migración del extremo abdominal a las bolsas escrotales de forma secuencial. Los autores citan las diversas complicaciones del extremo abdominal de las válvulas y en particular la asociación a hernia inguinal con o sin hidrocele. Tras exponer las diferentes hipótesis sugeridas en esta asociación, se resaltan ciertos aspectos clínicos-diagnósticos y puntualizan algunas consideraciones sobre el tratamiento quirúrgico de esta complicación.

Para los autores la existencia de un hidrocele en un niño portador de válvula VP debe de plantearnos como diagnóstico diferencial la migración del catéter a la bolsa escrotal.

Summary

After shunts hardware was developed around 1950, the standard treatment for hydrocephalus has been the placement of a ventriculoatrial or ventriculoperitoneal shunt. Ventriculoperitoneal (VP) shunt is the preferred procedure today. Although it is a safe and well tolerated method, many complications of VP shunting have been described in the literature. We are reporting the case of bilateral and sequential migration of the peritoneal catheter to scrotum in a 6-month-old infant. Complications related to the peritoneal catheter are reviewed and the ethiology and management of this complication are outlined.
