Resumen
Objetivos

Se presentan 18 casos de pacientes con tumores de base de cráneo a los que se les realizó resección craneofacial anterior durante el período de Enero de 1993 a Mayo de 1997. Se analizan el tratamiento quirúrgico y la morbi-mortalidad de estos casos.

Material y Métodos

18 pacientes (9 hombres; 9 mujeres), con edades comprendidas entre 16 y 68 años (edad media 47,3) presentaban tumores de la base craneal, 11 benignos y 7 malignos. A todos ellos se les realizó resección craneofacial anterior bien del sector central, lateral, o de ambos.

Resultados

Del total de pacientes, en 16 casos se obtuvo una resección completa. Las complicaciones inmediatas y tardías más frecuentes fueron las alteraciones oculares. Al final del estudio, 11 pacientes se encuentran libres de enfermedad. La mortalidad debida al tumor fue del 5,5%. El seguimiento medio fue de 29 meses con un rango entre 6–60 meses.

Conclusiones

La resección craneofacial es un buen método como tratamiento de los tumores de la base del cráneo. La mortalidad debida al tumor (5,5%) es aceptable y la morbilidad se relaciona directamente con la experiencia en este tipo de resección, siendo en nuestro caso similar con respecto a otras series.

Background

Eighteen patients with cranial base tumors underwent anterior craniofacial resection from January, 1993 to May 1997. We analyze the surgical treatment and the results.

Materials y Methods

Eigtheen patients (9 men; 9 women) of ages, ranging from 16 to 68 years (average = 47,3 year) were operated for cranial base tumors. Eleven tumors were classified as benign and seven as malignant. All of them underwent anterior craniofacial resection including either the central segment, the lateral segment (ofthe anterior cranial fossa), or both.

Results

In sixteen cases we performed complete tumor resection. The most frequent early and late complications, were ocular disturbance. At the end of the study (average follow-up = 29 months, range = 6–60 monts), eleven patients are alive, and free from recurrence. Tumor related mortality was 5,5%.

Conclusions

Craniofacial anterior resection is a good surgical procedure in the treatment of cranial base neoplasms. Surgical mortality related to the tumor (5,5%) is acceptable and morbility is directly related with the experience of the surgical team with this type of procedure. Results in our series are similar to those recently reported by other authors.
