Resumen

La hidatidosis osteomuscular es rara, estando afectado el raquis aproximadamente en el 50%. El diagnóstico preoperatorio es dificil y el tratamiento quirúrgico es asimismo problemático a causa de la invasión ósea. Presentamos el caso de un paciente de 36 años, dedicado a la agricultura, que aquejó lumbalgia y ciática bilateral. La TAC y RNM mostraron una gran masa quística desde T10 hasta L2 con extensión local al canal espinal y la 10ª costilla. El examen anatomopatológico postoperatorio confirmó el diagnóstico de equinococosis. El paciente fue operado de nuevo varios meses más tarde de otro quiste hidatídico de localización intrapulmonar.

Abstract

Osteo-muscular hydatidosis is rare, the spine being involved in about 50% of the cases. Preoperative diagnosis is difficult and surgical treatment is also problematic because of the invasion of the bone. In this report we present the case of a 36-year-old-farmer who complained about low back pain and bilateral sciatica. The CT scan and MRI showed a large paraspinal cystic mass extending from T10 to L2 with local spreading to the spinal canal and the 10th ribo The postoperative pathological evaluation confirmed the diagnosis of a echinococcal cyst. The patient was reoperated on sorne months later, with excision of another hidatid cyst of intrapulmonary location.
