Resumen

Los cavernomas representan del 5 al 13% de las anomalías vasculares del Sistema Nervioso Central. Su localización extra axial es rara y en el ángulo pontocerebeloso lo es más aún, habiéndose publicado solamente seis casos en la literatura accesible. Presentamos un nuevo caso con historia larga de afectación de los pares VII y VIII. En la Tomografía Computarizada (TAC) la lesión era ligeramente hiperdensa y no se realzaba con contraste. La Resonancia Magnética (RM) localizó precisamente la lesión. En imágenes ponderadas en TI era iso hipointensa con áreas moteadas hiperintensas y en imágenes ponderadas en T2 era heterogénea e hiperintensa con un ligero halo de hiposeñal rodeando la lesión. La paciente fue operada y la masa extra axial adherida a los pares VII y VIII Yal tronco cerebral fue extirpada en su. totalidad. La evolución fue sin complicaciones.

La Resonancia Magnética ha demostrado ser de gran utilidad para el diagnóstico de esta lesión.

Summary

Cavernomas represent from 5 to 13% of the vascular anomalies of the Central Nervous System (CNS). The extraaxial location of these vascular malformations is rare, and a location in the cerebellopontine angle (CPA) is even more so. Only six cases have previously been reported at this location. A new case of a patient with a long clinicai affectation of the VII and VIII cranial nerves is reported. On Computed Tomography the lesion was slightly hyperdense and did not enhance after contrast administration. Preoperative Magnetic Resonance Imaging provided precise indications of the anatomical location of the tumor. The lesion was iso-hypointense, with mottled hyperintense areas in TI weighted images and in T2 weighted images it was heterogeneous and hyperintense, with a slight surrounding rim of decreased signal intensity. The patient was operated and an extramedullar mass adherent to the 7th and 8th nerves and the brain stem, was totally removed. The postoperative course was uneventful.

MRI has been demonstrated to be very sensitive in the diagnosis of cavernomas. We present a case located at the cerebellopontine angle studied with MRI.
