Resumen

La monitorización de la presión intracraneal en el traumatismo craneoencefálico grave (TCEG) ha resultado un método de inestimable ayuda para la instauración de protocolos terapéuticos. En los últimos años, se han incorporado las mediciones de las diferencias arterio-yugular de oxígeno, del ácido láctico y del flujo sanguíneo cerebral para el tratamiento del TCEG al contribuir a un mejor reconocimiento del riesgo de inducir lesiones hipóxico-isquémicas asociadas. El uso de estas técnicas ha imprimido ciertos cambios en el esquema terapéutico. El presente estudio analiza el grado en que la monitorización de la saturación yugular de oxígeno (SjO2) condicionó modificaciones en la actitud terapéutica utilizada en 42 pacientes afectos de TCEG.

Summary

Intracranial pressure monitoring in severe head injury represents an important step in the design of therapy protocols. During the last years other monitoring modalities, such as the measurement of brain arteriovenus differences in oxygen and lactie acid concentrations, as well as measurements of cerebral blood flow have been introduced in order to improve early detection and prevention of brain hypoxic-ischemie lesions.

The aim of the present study was to analyse to what extent jugular venus oxygen saturation influenced therapy management in 42 severe head injury patients treated at our unit.
