Summary

Intracraneal arachnoid cysts are space-occupying benign lesions. Although congenital forms are found in younger patients, they can be diagnosed in adults and aged patients.

The midde cranial fossa is the most common location, left sided lesions are more frequent. Interhemispheric cysts in adults are very rare and only six cases have been previously reported.

We present a case of a large interhemispheric cyst in a old woman who presented a history of headache and dementia.

The authors review the cases previously reported and discuss the pathogenesis and management of symptomatic interhemispheric arachnoid cysts in the elderly.
