El uso de la derivación siringo-subaracnoidea en el tratamiento de la siringomielia ha sido ampliamente realizado. Presentamos nuestros resultados en una serie de 9 pacientes en quienes se utilizó dicho método. En 7 de ellos se encontró también malformación de Chiari. El seguimiento postoperatorio osciló de 1 a 6,3 años, siendo tanto la media como la mediana de 3,4. Los síntomas se estabilizaron en 4 pacientes, mejoraron en 3 y empeoraron en 2. El seguimiento mediante RNM mostró completa desaparición o reducción importante de la siringomielia en 6 pacientes, y cambios no significativos en 3. Los resultados clínicos no fueron absolutamente concordantes con los morfológicos, encontrándose casos con mejoría clínica sin cambios en la RNM y otros en los que la disminución de la cavidad siringomiélica no se correspondió con mejoría en los síntomas.

The use of syringosubarachnoid shunt in the treatment of syringomyelia has been widely practiced. We report our surgical results with this method in a series of 9 patients. In 7 of them, Chiari malformation was also found. The postoperative follow-up ranged from 1 to 6,3 years, the average and the median being of 3,4. Symptoms stabilized in 4 patients, improved in 3 and worsened in 2. The follow-up MRI examinations showed complete resolution or considerable shrinkage of the syrinx in 7 patients, and no significant changes in 3. The clinical results were not absolutely congruent with the morphological, as there were cases with clinical improvement without changes in the MRI, and others in wich the shrinkage of de syrinx was not associated with improvement of the symptoms.
