Resumen

Se presenta el caso de un varón de 22 años, portador de un sistema de derivación ventrículo-atrial y afecto de sepsis, tromboembolismo pulmonar de repetición e imagen ecocardiográfica compatible con un trombo grande en extremo atrial del catéter. Dado el riesgo de desprendimiento del trombo, se decidió su extracción bajo visión directa mediante auriculotomía derecha bajo circulación extracorpórea.

Se revisan las complicaciones de los sistemas de derivación ventrículo-atrial, haciendo especial referencia a las complicaciones tromboembólicas.

Summary

A 22 years old man carrier of a ventriculo-atrial shunt presented with pulmonary thromboembolism, septicemia and a thrombus at the tip of the catheter. Due to the risk of massive embolization during a blind withdrawal of the catheter, it was removed by an open technique under cardiopulmonary bypass.

A review of the complications of the ventriculoatrial shunts are made with special reference to thromboembolic complications.
