Paraplejia secundaria a expansión vertebral metastásica. Caso clínico

Resumen

Presentamos el caso de un paciente de 68 años de edad que ingresó con una paraparesia muy avanzada producida por una metástasis ósea de un carcinoma de próstata diseminado.

La lesión vertebral era puramente intraósea y el mecanismo de producción de la paraplejia fue la estenosis provocada por la expansión de la propia vértebra (cuerpo y arco neural) en el canal espinal. Pasados 5 años desde la intervención quirúrgica el paciente -recuperado totalmente de su paraplejia- se mantiene en buen estado clínico y activo.

Resulta lógico pensar que este mecanismo de afectación vertebral metastásica no se hubiera beneficiado del tratamiento radioterápico.

Summary

We are reporting the case of a 68 year-old white male admitted to our Hospital with a severe paraparesis caused by spinal metastasis of a prostatic carcinoma. As the tumor was purely intraosseous we think that the stenosis produced by the insuflation of the bone was the mechanism by which toe patient became paraplegic. Five years after surgery the patient has fully recovered from paraplegia and is doing well.

We think that radiotherapy has little to offer on this type of spinal cord compromise.
