Resumen

El mucocele esfenoidal es una lesión quística de naturaleza benigna, pero con un importante potencial de destrucción local. Su apariencia clínica y radiológica puede simular otros procesos más frecuentes en esta región, como el adenoma hipofisario, fibromas nasales y metástasis. Los síntomas neurológicos son infrecuentes en los ciento cincuenta y cinco casos publicados previamente.

Los autores aportan dos casos de mucocele esfenoidal que debutaron con diplopia y cefalea. Los estudios neurorradiológicos, incluyendo RMN y angiografía, no fueron de ayuda para identificar preoperatoriamente la naturaleza de la lesión. En ambos casos se practicó un drenaje transesfenoidal del quiste. En el cultivo del material mucoso creció un Streptococcus sp en el primer caso y un aspergillus en el segundo.

Se destacan la benignidad del proceso, las dificultades del diagnóstico diferencial y su buena resolución tras la intervención quirúrgica por vía transesfenoidal.

Summary

Mucoceles of the paranasal sinuses are cystic lesions of benign nature but with a high local destructive potentiaI. Clínical and radiological appearance may mimic other more frequent diseases of this region as pituitary adenomas, nasal fibromas and metastasis. Neurological symptoms were infrequent in the one hundred and fifty cases previously published.

The authors are reporting on two patients with sphenoidal mucocele who complained both of headache and oculomotor nerves paresis. The neuroradiological studies, including MRI and Angiography, did not help to identify preoperatively the nature of the lesion. Both cases underwent transsphenoidal drainage of the cysts. Cultures from the mucous material grew Streptococcus sp in the first case and aspergillus in the second one. The oculomotor disturbances resolved in both cases following operation.

The authors emphasize the benign nature of these cysts, the difficulties in the preoperative diagnosis and the effectiveness of surgical treatment.
