Estudio comparativo de los resultados quirúrgicos y los cambios en resonancia magnética en 29 pacientes con siringomielia

Resumen

Los autores presentan una serie de 29 pacientes con siringomielia tratados quirúrgicamente mediante descompresión craneovertebral (DCV) (19 casos), derivación quistoperitoneal (DQP) (9 casos) y derivación ventriculoperitoneal (DVP) (1 caso), en la que se comparan los resultados clínicos y de control por resonancia magnética.

La presentación clínica más frecuente consistió en alteraciones sensitivas, dolor ocdpitocervical y paraparesia. El tratamiento quirúrgico es más efectivo para resolver el dolor y el defecto motor (50% con DCV y 30% con DQP), que las alteraciones de sensibilidad.

La cavidad quística se resolvió en un 90% de los casos existiendo una disociación entre los porcentajes de mejoría clínica y los de imagen, especialmente en los sometidos a DQP.

La DVP fue efectiva clínica y radiológicamente en el paciente con complejo siringomielia-Chiari-hidrocefalia comunicante.

La cirugía DCV puede ser usada como primera elección en pacientes con siringomielia, reservando la DQP para los casos en los que no se consigue reducir la cavidad medular.

Summary

The authors present the clinical and control MRI results in 29 patients with syringomyelia surgically treated by craniovertebral decompression (CVD) (19 cases), syringoperitoneal shunt (SPS) (9 cases) and ventriculoperitoneal shunt (VPS) (1 case).

Sensory impairment, occipito-cervical pain and paraparesis were the most frequent clinical features. Surgical procedures were more effective on pain control and motor deficit restoration (50% with CVD and 30% with SPS), than on controlling sensory disturbanees.

The syringeal cavity disappeared in 90% of the cases on MRI control after surgery, but there was not correlation between clinical improvement and syrinx coIlapse, specially after SPS.

Syringomyelia-Chiari-Comunicating hydrocephalus complex was successfully treated with VPS.

Craniovertebral decompression should be used as the first surgical technic for syringomyelia, while SPS may be performed in cases in which the syringeal cavity is still large.
