Resumen

El tratamiento de los traumatismos raqui-medulares plantea en la actualidad importantes controversias entre los partidarios del tratamiento conservador y el quirúrgico.

En el presente trabajo se analizan los datos clínicos, etiopatogénicos y terapéuticos de 56 casos consecutivos de trauma espinal y se comparan los resultados con los publicados en la literatura.

Del presente estudio se desprende que los varones con edades alrededor de los 36 años constituyen la población de mayor riesgo y que la causa fundamental son los accidentes de tráfico. El tratamiento quirúrgico de urgencia raramente es necesario y, en la mayor parte de los casos, está contraindicado. El tratamiento quirúrgico puede aconsejarse en aquellos casos en que el estado del paciente haga prever las mínimas complicaciones posibles y debe ser realizada por equipos expertos.

The treatment of the spinal trauma is currently a matter of controversy between different authors.

In this paper, the results in a series of 56 patients with spinal trauma are presented and discussed. The etiopathogenic factors, the clinical picture, the therapeutic approaches, and the follow-up were taken into account and compared with those published in the Iiterature. It is concluded that men over 36 years are mainly affected. A car accident is the cause of the trauma in the majority of the cases. Surgical management is seldom required in the acute fase and, in few cases, it may be contraindicated. The surgical treatment is indicated in those patients with a good general condition, in whom the possible complications are usually minimal. In any case, the operation must be carried out by expert surgical teams.
