Summary

Spinal cysts represent a rare cause of spinal cord and nerve root compression.

We report a case of sacrum intradural arachnoid cyst presenting with low-back pain radiating down the left leg 20 years after a penetrating injury to the sacral spine. The diagnosis was verified by computed tomography and magnetic resonance imaging.

The etiological and pathological features are discussed. We concluded that RM is the modality of choice in the demonstration of spinal arachnoid cysts.
