Resumen

La aparición de transtomos del habla secundariamente a la cirugía de los tumores de la fosa craneal posterior, es poco habitual, y aparece escasamente recogido en la literatura. Se presenta el caso de una niña de 3 años, que tras extirpación de un meduloblastoma de cerebelo, presentó a las pocas horas un mutismo completo que tardó 5 semanas en recuperar. La ausencia de alteraciones de vías largas y pares craneales, hacen pensar en una posible causa cerebelosa. Se revisan los mecanismos propuestos para explicar el cuadro clínico así como los 13 casos publicados con anterioridad.

Summary

Total absence of speech after posterior fossa surgery is a rare complication. We present a case of a 3 year-old child with mutism of cerebellar origin occurring a few hours after the extirpation of a large vermian and 4th ventricular medulloblastoma. The recovery of speech started to appear 5 weeks postoperatively. The absence of affection to long tracts and nuclear cranial nerve palsies exclude the possibility of a pseudobulbar palsy. The literature concerning physiopathological mechanisms and 13 previously described cases is reviewed.
