Resumen

Los hematomas subdurales agudos interhemisféricos, son una complicación poco frecuente de los traumatismos craneoencefálicos, habiéndose descrito alrededor de 100 casos desde 1940. La forma habitual de presentación es una hemiparesia contralateral con una mayor afectación de la extremidad inferior.

Presentamos tres casos de hematomas subdurales agudos interhemisféricos de origen traumático atendidos en nuestro servicio y realizamos una revisión de los casos publicados anteriormente, con una mayor atención a las opciones del tratamiento. En la mayoría de los casos, una actitud conservadora con un control estricto del paciente es la alternativa terapéutica de elección sobre todo en aquellos pacientes sin deterioro importante del nivel de conciencia, relegando la actuación quirúrgica a los pacientes con mayor afectación neurológica.

Summary

Interhemisferic subdural hematoma in adults is a rare complication of head injury with no more than 100 cases reported since 1940. The classical presentation of this disorder is a contralateral monoparesis of the leg or a hemiparesis more pronnonced in the leg.

We report three cases of interhemisferic subdural hematoma following closed head injuries and the review of the literature with an analysisande of the therapeutic possibilities. Conservative management under close supervision may be a proper attitude in those patients without alterations of consciousness. Surgical treatment is necessary in patients with progressive deterioration.
