Resumen

Se describen los casos de seis pacientes con raíz nerviosa conjunta diagnosticados todos ellos durante el acto quirúrgico y que habían sido diagnosticados preoperatoriamente de hernias discales. En cinco casos se utilizó RMN en el diagnóstico preoperatorio y en un caso la Tac-lumbar.

El tratamiento quirúrgico fue realizado mediante fenestración y amplia foraminotomía en cinco casos y un caso fue tratado con laminectomía y foraminotomía. Se realizó exéresis de hernia discal en cinco casos.

El seguimiento clínico fue de 4 meses a 3 años, en cuatro casos el resultado fue excelente, en un caso bueno y en otro el resultado fue regular.

Se comenta la relativa rareza, la patogenesis, los aspectos diagnósticos y terapéuticos.

Summary

We describe six cases of conjoined nerve roots detected during the surgical procedure. Preoperatively the patients were diagnosed of herniated disk. In five cases MRI was performed, and lumbar-CT as used in one case. The surgical treatment consisted of fenestration and wide foraminotomy in five cases, and laminectomy and foraminotomy in one case. The excision of disk herniation was achived in five cases. All patients were followed for a minimum of 4 months and some for three years. The clinical result was excellent in four cases, good in one case, and poor in one case.

The relative rarety, pathogenesis, diagnosis and therapeutic aspects of this entity are discussed.
