Resumen

La inestabilidad cervical postraumática crónica ha sido tratada casi siempre con medidas conservadoras. Hoy en día, cuando éstas fracasan, el tratamiento quirúrgico mediante abordaje posterior tiene su papel para estabilizar la columna cervical.

Presentamos un grupo de 31 pacientes, de predominio femenino y edad media de 27,5 años, afectos de inestabilidad cervical postraumática crónica de origen ligamentario donde se practicó fijación instrumentada por vía posterior mediante tornillos anclados en los macizos articulares.

En el 93,5% de los casos remitió la clínica previa. La lordosis fisiológica se corrigió en todos salvo en uno. El 77,4% se han reintegrado hasta el momento a su actividad laboral previa. No se registraron complicaciones.

Los resultados demuestran que la fijación posterior transarticular es una técnica segura y eficaz para el manejo de este tipo de inestabilidad representando un avance sobre otros procedimientos.

Summary

Postraumatic chronic cervical instability has almost always been treated by conservative measures. Nowadays when these methods fail, surgical treatment by posterior approach has his role in order to stabilize the cervical spine.

We present a group of 31 patients with female preponderance and 27.5 years of mean age affected of postraumatic chronic cervical instability of ligamentous origin treated by cervical fixation through articular mass screws and plates.

Clinical remission was reached in 93.5%. Physiologic cervical lordosis was corrected in all except in one case. 77.4% of patients have returnned to work. No complications were detected.

Our results showed that lateral mass plating is a safe, effective technique for managing this kind of instability that represents a major advance over other procedures.
