Image-directed stereotactic thalamotomy for the treatment of Parkinson's disease

Summary

Fourteen image-directed stereotactic thalamotomies have been performed in two years on ten patients with Parkinson's disease. An isocentered system for functional stereotactic procedures with the Cosman-Roberts-Wells frame and a CT localizer that allows extrapolation of target data directly from the CT slice has been used. Sucessful targeting was verified by intraoperative electrical stimulation and postoperative CT scan. The lesions have been made by thermocoagulation. Complete reductíon of the symptoms treated was observed in four patients, in two cases the symptoms have not changed. There were no operation related complications. This image-directed stereotactic thalamotomy is safer and less distressing for the patients than previous radiological procedures, its physiological accuracy is similar and it is available to many units with the C.R.W. frame.

Resumen

Se han realizado catorce talamotomías estereotáxicas basadas en el diagnóstico por imagen en diez enfermos a lo largo de dos años, estos enfermos padecían la enfermedad de Parkinson. Se ha empleado un sistema isocéntrico con la guía CRW y un localizador en la TAC que permite la obtención directa de las coordenadas a partir de la imagen de la TAC. La exactitud de las coordenadas se ha verificado mediante estimulación eléctrica intraoperatoria y TACs postoperatorios. Las lesiones se han efectuado mediante termocoagulación. Se observó una mejoría total de los síntomas en cuatro enfermos, en otros cuatro se observó una mejoría importante y dos casos no mostraron cambios. No han existido complicaciones postoperatorias. Esta técnica de talamotomía dirigida mediante diagnóstico por imagen es más segura y tolerable para los enfermos que los procedimientos radiológicos previos, la exactitud fisiológica es similar y posibilita la realización de estas técnicas por servicios con la guía CRW.
