Resumen

Se realiza un estudio sobre el funcionamiento del quirófano principal para la cirugía programada, durante el año 1998, en el Servicio de Neurocirugía del Hospital Puerta del Mar, de Cádiz, integrado en el Servicio Andaluz de Salud (S.A.S.).

A partir de los datos obtenidos en los partes de quirófano, rellenados por el persnal de Enfermería, ciñéndose a los datos reales (lo que no ocurre con muchos otros Servicios Quirúrgicos del centro), se presentan distintos aspectos comparativos de la labor realizada, especialmente el aprovechamiento, el tiempo perdido, y el utilizado para otras funciones, como limpieza, etc. Asimismo, se repasa la duración media de los procedimientos operatorios más frecuentes y ciertas variaciones que dependen de los diferentes neurocirujanos. Se ha procurado no hacer juicios de valor, pero no se han omitido los aspectos negativos, como las intervenciones suspendidas sin justificación suficiente a juicio del autor.

Parece desprenderse de este estudio que, a pesar de que el Servicio de Neurocirugía, si nos atenemos a las estadísticas, es uno de los más efectivos del Hospital, se debería obtener un mayor aprovechamiento de los recursos.

Se intenta suscitar un cambio de opiniones entre los Servicios Neuroquirúrgicos de los distintos Hospitales del país, la mayoría integrados en sistemas similares a los nuestros, y analizar las posibles ventajas y deficiencias de los diferentes sistemas de gestión; en este caso la Sanidad Pública.

Summary

The performance of the main neurosurgery operating room for scheduled neurosurgical operations is studied during the 98 period at Puerta del Mar Hospital. This centre is integrated in the S.A.S. (Andalusian Health Service).

All data used for calculations are obtained from the reports filled by the nursery personnel after each neurosurgical operation.

Some different aspects are shown trying to evaluate the work done, specially focused on efficiency, wasted time and time spent for different maintenance tasks and cleaning activities.

The article includes also average duration of regular surgical procedures and its deviations as a function of the main surgeon.

This work does not pretend to establish who does better, but negative aspects like cancelled operations due to wrong decisions (always following author's criteria).

It seems that the neurosurgical service effectiveness is, according to the statistical analysis, one of the best in the group of services of the subject center, but the study also shows that better resource exploitation can be accomplish.

The article goal is to establish a discussion between the different spanish neurosurgical services (a lot of them integrated in their regional Health Services similar to S.A.S.) in order to compare different management systems and their advantages and obstaeles.
